Offset flange question

I everyone I need some help with this plumbing in my basement washroom.

I'm trying to install an offset flange toilet. After I broke the concrete i found some problems. If you see the pictures the rough-pipe is 4" and my flange is also a 4", so I do not which is my mistake.



The offset flanges comes in different types of measures?.



How can I solve this?
